The Lumenos plans will be high deductible plans and include some versions featuring reward dollars for healthy activities as well as full coverage for preventative care before the deductible.
UHC & PacifiCare

PacAdvantage Closing Shop

The reason? Blue Shield of California pulled out of the plan for economic reasons and Health Net followed suit. The remaining small HMO plans could not continue in PacAdvantage without the larger carriers and so the whole program collapsed.
State Mini-COBRA Programs
Many states offer mini-COBRA programs for groups under 20 employees. I have added a list on my site of current mini-COBRA states. You can access the page here.
